Certificate of Fitness - Tackle Players 40 years+

A person who has reached the age of 40 years or is reaching that age during a football year and desires to register and participate in contact Rugby League under NSWRL jurisdiction must provide to the relevant registering club the completed certificate of fitness using the approved NSWRL form prior to participating in any game, training session or carnival. 

 

The certificate of fitness form must be completed by a registered Medical Practitioner in Australia stating:

a) that the person is medically fit to participate in body contact sport; and 

b) such other qualifications, terms and conditions as may be required by the NSWRL insurance provider as part of the registration process.

 

The completed NSWRL certificate of fitness form is to be uploaded to the players My Sideline registration profile.
